As a new patient we want to welcome you to Performance Physical Therapy. As a provider of physical therapy services for Northern Virginia we have highly qualified staff to meet all of your needs.

On your first visit please come prepared with the following items:

Government Issue Photo ID
Doctors Prescription (If applicable)
Completed New Patient Paperwork
